  • The employee’s date of hire;
  • The employee’s date of termination;
  • The approximate number of employees the employer employs (If the employer is a large, national corporation, a statement to this effect will suffice)
  • The employee’s most recent salary or hourly rate of pay;
  • Whether the employee had any sort of employment contract stating any termination had to be for good cause;
  • The reason the employer gave for the termination;
  • What the employee alleges are the real reasons behind the termination;
  • What evidence the employee might use to support his/her allegations; and
  • Whether the employee has filed with any state or federal agency a complaint regarding the termination.
