  • Whether the employee still works for the employer;
  • The approximate number of employees the employer employs (If the employer is a large, national corporation, a statement to this effect will suffice);
  • The employee’s most recent salary or hourly rate of pay;
  • A description of the conduct and/or statements the employee views as sexual harassment;
  • Whether the employee reported to management the harassment;
  • If the employee did report the harassment, management’s response.
